If you would like to get a headstart on the credentialing process, please start with the link that says "Initial Request For Privileges." On the first page of that initial request for privileges packet, there is a detailed listing of what you need to do to be credentialed at one of our Ambulatory Surgery Centers. Pay special attention on how to fill out the Texas Standardization Credentialing Application electronically. Doing this correctly the first time will save you a lot of time in the future. On the second page of initial request for privileges packet there is a comprehensive checklist that you'll need to complete.
